Mentor Confirmation Forms

All students applying for funding must have their mentor complete the appropriate form for their award.
The deadline to submit this forms is the same as for the awards.

Mentor/Mentee Expectations Document

This agreement establishes the expectations for undergraduate students and their 51�� mentor as a guide for the student's research and/or scholarly activity experience within projects funded by the Office of Undergraduate Research and Scholarly Activity (URSA). This agreement forms the basis for assessment of progress and evaluation of outcomes, either formally or informally, and to ensure that the needs of the student and mentor are being met during the project period.

Directions: The student and mentor should complete this agreement together using the template to facilitate your conversation as you establish and clarify expectations for communications, milestones, and outcomes. Fill it out at the beginning of the project and refer to it as needed during the project term. Use the agreement to (1) discuss and reflect on the student’s progress, (2) ensure that the mentor is meeting the student’s needs, and (3) assess the mentee-mentor experience and relationship. The student and mentor should retain a copy of the agreement throughout the term of their project.

All students receiving URSA awards are required to complete the a reflection form (evaluation) before the end of the semester when they received their awards.

Student Travel Award Reflection Forms

All students receiving URSA awards are required to complete the a reflection form (evaluation) before the end of the semester when they received their awards:

AY 2022-2023 Deadlines
- Summer 2023: Aug. 11, 2023

AY 2023-2024 Deadlines
- Fall 2023: December 10, 2023
- Spring 2024: April 28, 2024
- Summer 2024: August 11, 2024

- For both individual and group travel
- For students receiving virtual registration travel funds

Internship Award Reflection Form

All students receiving URSA awards are required to complete the a reflection form (evaluation) at the end of their award period:

Reflections for this award will be due within two weeks of the completion of the internship.
- Specific reflection due dates will be communicated in the award paperwork upon awarding.
